As an accredited provider of Continuing Medical Education (CME), Essentia Health is required to comply with ACCME Standards for Commercial Support.
Everyone in a position to control the content of an educational activity will disclose to us all relevant financial relationships with any commercial interest. If a conflict of interest exists as a result of a financial relationship you may have, this needs to be resolved prior to the activity. Complete the Faculty Disclosure Form and return it to us as soon as possible at the contact information listed below. It will remain valid for one year. This information is necessary in order for us to be able to move to the next steps in planning the CME activity. If you refuse to disclose relevant financial relationships, you will be disqualified from being a part of the planning and implementation of the CME activity.
The intent of this process is not to prevent a speaker with a potential conflict of interest from making a presentation. It is merely intended that any potential conflict should be resolved prior to the activity as required by ACCME policies. These policies require that your presentation is free of commercial bias and that any information regarding commercial products/services be based on scientific methods generally accepted by the medical community. When discussing therapeutic options, please use only generic names. If it is necessary to use a trade name, then those of several companies must be used.
